Clinical Standards NHS QIS – Gill Dennes on this group for COPD clinical standards. Planning standard for spring 2010
Asthma care group – schools very bad, kids have poor understanding of their problem. Research project happening in Glenrothes area – Anne McKean involved. P5 – P7 and teens, all schools contacted re this in Glenrothes area – response from only from 4 children.
Op Division Meetings – still held quarterly rotating between the Victoria Hospital and the Queen Margaret Hospital

Nurses who stated interest at last meeting are still waiting to find out if their time will be backfilled. Nurses are still interested though.
Update from NMP Advisor
Discussed CPD meetings and peer review events – feedback from all events is excellent. NMP Advisor post is ongoing until March 09. Waiting to find out if contract will be renewed.
Frequently asked questions document produced by Colette is to be published and circulated throughout NHS Fife.
Colette advised group that a nurse has been struck off the register for inappropriate prescribing.
Actions from previous meetings
Response from Anne Ritchie read out and circulated to group re spirometry in children and if there are any documented predicted values that could be used when carrying out this test. Anne advises that nurses in primary care should not be doing spirometry in children – Anne McKean paediatric nurse specialist felt that primary care nurses were well placed to carry out this test in primary care.
Gill Dennes is organising a spirometry meeting with Lena Stewart from A&H and will be including spirometry in children as a topic.
Susan Chapman advised group about a website that can be used to find predicted values of spirometry for children over 8 – www.spiroexpert.co.uk or .com
Can order paediatric mouthpieces for spirometer through stores.
Karen Lee has made contact with Molly Courtney and she would be pleased to come along to our next meeting on the 7th of April 2009.

Any other business
Anne McKean brought up the fact that she has noticed that Singulair being used in paediatric patients has been causing changes in behaviour in some cases. Karen Lee and Lena Stewart both advised the group that if they felt that this was a side effect or an adverse effect from using this product then this was a yellow card event and should be reported as such.

Update on any other meetings
Nicky Credland updated group on Diabetic MCN meeting. Sharps policy is currently being updated. Will initially be for Dunfermline and West Fife CHP, with the intention of roll out to NHS Fife. Tessa Kidd has been writing policy – as sharps bins not prescribed for diabetic patients which means that there is a Health and Safety issue with sharps used by diabetics [sharps are put into general household rubbish]. It will be the responsibility of whoever prescribes the sharps to ensure that provision for safe disposal is provided. A new patient leaflet has been developed. 1 litre sharp bin to be prescribed. Each practice requires a SEPA certificate.
Wendy updated the group that there are now two AHP prescribers – namely Karen and Wendy – and Fiona is studying at the moment. Discussion about courses available at different universities. Also discussed Supplementary Prescribing course vs. the independent prescriber’s course.
Colette advised the group that the Fife Prescribers Website has been updated with new links & AHPs [recently qualified] details could be added to website.
All student prescribers will meet to discuss shadowing – Colette to organise.
Wendy discussed anecdotal evidence that patients are satisfied when CDM clinics are nurse led.
24th November – 9am to 4pm, Respiratory Meeting at Dean Park Hotel, Kirkcaldy. Looking at new asthma guidelines and will cover the topic of Spirometry in children.
17th November [evening] and 20th November [afternoon] Respiratory meetings being run by NHS Fife. Same programme being run twice. All non medical prescribers should have received an invitation to attend one of these meetings – can attend the one that suits them best.
Actions from previous meeting
Karen Lee has contacted Andy Robson [lung function department] at Edinburgh Royal Infirmary re Spirometry in Children. No response as yet. Also contacted Anne Ritchie re the same – again no response as yet. Karen has been asked to e mail the group when she has a response rather than wait until the next meeting to discuss it.
Karen is also waiting on dates for Molly Courtney to come to meeting. Group agreed that spring would be a good time for her to come to the meeting.
Discussed X Ray challenges again. Still no further forward with this. Course completed by many, but no guidance as when they can start referring as yet.
Fife Formulary – nurse Rx input
Question asked re would anyone like to attend the Fife Formulary Development Group meeting. Primary care representation is needed. The meeting lasts for about 2 hours, and is held every 6 – 8 weeks. Heather Sheddon, Michelle Horne, Gaynor Black, and Lorraine Summers are all interested – issues were raised about using their clinical time to attend a meeting. Colette will discuss with NHS Fife re if the practice could be reimbursed to allow them to attend before any decisions are made about who would be keen to attend.
What’s on your mind?
Barcodes on Prescriptions – question asked if this will be introduced for all NMPs. Yes was the answer – some systems e.g. emiss already have this on their system.
Emiss being introduced in some practices – St Andrews have just had it installed. Good points raised so far are BNF live on system, good patient information leaflets, emis templates can be changed – it also seems that the information that is input into system drills through all the various screens.
Heather enquired about paperless/paperlight practice when awaiting for notes. Same challenges faced in secondary care – concerns about awaiting notes and sending letters to surgery – this can take up to 2 weeks. Most concerning is issues with stopping drugs when in hospital and not clear when / if they need to be restarted.
Awaiting a possible system for everyone to access notes from primary and secondary care.

Meeting opened and delegates welcomed. Thanks were given to Servier, Recordati and Boehringer for their financial support of the meeting.

Minutes were accepted as accurate by those present.
Update on any other meetings
Jacquie reported that the non-medical prescriber’s acute meeting had discussed the policy that is now in place. Line managers require written information on prescriber’s competencies. It is hoped that by the time the policy is updated a proforma will be included that will enable continuity of competency acknowledgement. Colette confirmed that the policy is due to be updated in 2010.
Heather commented that Gill Dennes will be organising a study day related to new Asthma guidelines that are due in December. Discussion then occurred about spirometry in children. Problems were highlighted that there were no paediatric guidance vitalograph values. After further discussions advice was given to either refer to specialist- contact respiratory technicians who deal with the machinery used and that other delegates would take it back to practice and discuss it personally. This highlighted training issues on paediatric investigation. This should be helpful with the proposed or future training events. Colette stated that she is providing a study event around paediatric examination; she was going to discuss this later in the meeting.
Susan stated that ‘Byetta’ has now been taken to the Area and Drugs and Therapeutic Committee to be prescribed by practice nurses and GP’s who have the competence.
Rosiglitazone, Nicky and Susan debated the clarity of the guidelines. It was stated that new guidelines due will ensure that prescribing this drug will become clearer.
X-ray
Colette stated that we are awaiting the XR referral policy. There are RCN guidelines out for nurses requesting x-rays. Lorna will discuss this at the Dunfermline Respiratory task group meeting next week.
Update from Colette Henderson
Colette stated that there will guidance issued soon re prescribing for syringe drivers. This is related to the mixing of drugs being administered. Colette has said that all non medical prescribers will receive the guidance. Discussions took place around this subject. As always prescribers awareness of compatibility of drugs is paramount when prescribing.
Colette has put together a booklet on frequently asked questions for the non-medical prescriber. It includes information on medicines management in NHS Fife, this will be published soon.
A CPD event to be organised in November 2008 - week of 17th. Plans to discuss respiratory care. It is hoped that a future events will include wound care. Sheila has agreed to assist with giving names of potential facilitators. Thought for February 2009 CPD event to be on pain and pain management.
What’s on your mind?
Nicky then asked if fellow prescribers keep a record of all numbers on each prescription sheets. Good practice must be adhered to and this is detailed in the blue Non Medical Prescribing in Scotland folder that everyone should have access to. Nicky added that she has come across issues when printing computer scripts it is not always her name that comes on to Gpass scripts it was thought that she should discuss this with her practice manager as this is only because the computer has been set up to do this. Discussions around pad safety showed varied safety precautions taken by prescribers, who all appear to be practising safely.

Update on any other meetings
Feedback from Sharon Kirk re ANP conference. Poor attendance this year. Workshops were good.
Dunfermline and West Fife Respiratory Group has had meeting – guidance being rolled out to Fife regarding Dry powder: advice recommends 1st line easihaler in place of pulvinal. Not looking at big switches. Group has also looked at COPD action plans. All practices should have received them by now. Good feedback on these as they can be adapted for the individual patients needs.
Review of Educational Meetings – May
Good discussion around both meetings. Feedback was good, comments were all constructive. Planning on having legal issues discussed on an annual basis. Issue raised was that some people missed from the distribution list. Colette to address this. Next educational meeting is in planning phase – and everyone will be notified in due course of dates and topics.
Monday 23rd of June, and again on Friday 27th June Andrew Jamieson was chairing a meeting re rosiglitazone and ? the need to stop using this drug. Much discussion around research required for glitazones
Any other business
Discussion around Molly Courtney attending meeting. It was felt that this would be appropriate, and it would be useful to hear her present to a smaller interactive audience.
Further talk around nurse x ray referrals - questions raised around nurses referring out with Fife e.g. nurses in the East Neuk refer to Dundee, and how this will work.
Susan Chapman recommended the Charles Bloe training for X Rays, costs £15, internet based and is a self directed online package.
Sharon Kirk recommended the Charles Bloe training for blood investigations again this is an online internet based, self directed learning package.
Some discussion around the NES Paediatric Training – some present felt that it was too basic. Colette to find out if Practice Nurses could access this and will feedback at the next meeting.
Discussion around training for PNs and funding of events. Nurses could approach reps for funding.
Heather Sheddon asked group what was being prescribed by nurses for exacerbations of COPD and the question was asked “Do we need guidance for steroids?” Gill Dennes will feed this back to the Resp MCN and then back to the group
Practice Nurse payscales were discussed.
General questions around prescribing in practice. Susan Chapman recommends discussing personal prescribing with the CHP pharmacist who will print off your prescribing stats. There are challenges quantifying true figures as repeat prescriptions may be signed by GPs after initiation by non medical prescribers.
Peak flow readings v spirometry in paediatrics. If Peff is persistently low try spirometry – it’s in the new guidelines.
Suggested websites for checking out were www.testandcal.com for numeracy tests for anyone undertaking non medical prescribing modules.
GPIAG are to be putting the new asthma guidelines on their site, with the main changes summarised and are also planning on producing aide memoirs, and updated summary sheets. These should be published in the summer.

Update on any other meetings
Secondary Care prescribing meeting at Victoria Hospital coincided with changeover SHO rota. Better attendance than previous meetings. Meetings have been set so that half are held at the Queen Margaret site, and the other half at the Victoria hospital.
The Association of Nurse Prescribers conference is being held again this year at the Carnegie Conference Centre on 23.4.08 – this coincides with the Equip meetings in West Fife. K.L. asked if anyone attending could make notes for future feedback to the group meeting in May.
Update on Nurse Representation on committees
N.M.P’s attending / representing on various groups – trying to collate what groups and who is attending / representing.
Karen Martin represents NMPs on a West Fife Prescribing meeting. Colette gave a short presentation to explain the purpose and remit of the NMP working group and how they fit in to the ADTC meetings.
Colette stated that at present there is not a flow chart available for NMPs to see who would be attending/representing at ADTCs.
Jill Logan attends Glenrothes/East Fife meetings. Jill made the point that many of the items discussed at these meetings are not relevant to NMP “per se”. But there is opportunity to raise the profile of NMP in general on these committees.
It was felt that it would be appropriate to go ahead with sending out questionnaire to Non Medical Prescribers in Forum to collate what information we currently have and start to put together the organisational chart regarding which nurses are on what committees. Karen Lee to action this.
Discussion ensued regarding Allied Health Professionals [Podiatry] and the process of supplementary prescribing and how difficult this may be to implement in certain areas such as acute. It was agreed that supplementary prescribing is still advantageous for new prescribers to gain confidence and act as a psychological safety net. Or moving into new areas of practice etc.
Fife Formulary – nurse Rx input
Discussion around how this impacts on NMPs. Issues surrounding prescribing cost effectiveness and set amounts used in electronic prescribing.
Question asked if there is a mechanism where Fife P forum highlight problem areas / disseminate information. Group agreed to having protected time to discuss different therapeutic areas and for September Meeting it was agreed that Respiratory Medication on the Fife Formulary will be discussed. This discussion will be facilitated by Jill Logan. New guidelines for COPD and Asthma should be included in this discussion.
The group stated that different therapeutic areas of discussion are of interest to different disciplines due to a holistic approach that nursing / AHPs undertake.
Update from NMP Advisor
Update from Colette Henderson who has now been in post for 3 months. She is working in an advisory role for NMPs – developing and facilitating CPD events and disseminating this information Fife wide.
Colette at present still involved in developing programme of CPD events and will be hoping to have this ready soon. 1st session – Fife Policy on NM prescribing & legal issues.
Other areas such as Mental Health / Paediatrics, community nurse prescribing – will be included in programme. Colette has asked the group if they would be agreeable to help provide input into any future CPD events. This was agreed by those present.
Mental Health Forum for Nurse Prescribing would be a good resource for CPD events dealing with mental health. Fiona Milligan, MH practitioner will be leaving post shortly thanked everyone for all the support this forum has provided in her role of NMP.
Government Policies.
Safe Prescription Document – Patient information on NMPs - Any comments to go direct to Colette who will feed back on this document.
Catriona Marshall highlighted difficulties with GPASS F3 Facility. Apparently NMPs were not included in new updated F3 facility. Catriona was asking if Colette could take this forward. Discussion around using 2 GPASS open windows and switch between for Medical Prescribing and NMP if required.
Any other business
Podiatry NMP students asking for opportunity to “shadow” experienced prescribers. Directed to Website to look at areas and willing prescribers to arrange this. New website for clinical management plans at cmponline.co.uk. This is a library of CMPs categorised by drug / area etc.

Update on any other meetings
1. Nothing to report on any other Prescribing meetings that have been held in primary care.
Acute meetings being held now on alternate sites [Victoria then Queen Margaret]. Attendance made by the same HCPs at all meetings – comments made that they would like attendance to be mandatory. It also seems that many are not included on the e mail distribution list – acute nurses have agreed to address this to ensure that everyone is included on the list.
2. Comments made around non medical prescriber representation made on the Area Drugs and Therapeutic Committee, and if there are representation from nurse prescribers on any local CHP prescribing groups. It was suggested that this could be set out on a tree format so that everyone is aware what is happening both in primary and secondary care – and who represents the non medical prescribers on these groups. Karen Lee to action this.
Document for Consultation
Main business of the evening was to discuss the latest document for consultation - “A safe Prescription”, collate the groups comments and submit them as per requested – as a representation of the groups thoughts on the paper. Michelle Horne collated the group’s comments, and will submit them in a timely manner – they will then be posted on the Web site – for all members to see.
Next Meeting – date
Date of the next meeting was set for Wednesday 26th March 2008 – venue to be the Dunnikier House Hotel. Buffet supper at 6.30pm with the meeting commencing at around 7pm. Communication re this will be sent out during March.
Meetings 2008 / educational & forum
This will now be part of Colette Henderson’s Role in supporting the Continued Professional Development of Nurse Prescribers in Fife – and she will be developing an education programme over time.
The group felt that it would be appropriate to put together a bank of speakers that they have already heard, and felt were excellent – so that Colette may draw from this if appropriate. Karen Lee to collate this information.
Carole Morrison indicated that it is hoped that this education programme will be implemented by March 2008.
This programme will be designed so that individuals can dip in and out of it depending on what they consider their needs to be.
Group keen that specific areas are addressed e.g. paediatrics and other specialist areas.
Any other business
1. The group was keen to consider the Fife Formulary – in the areas in which they prescribe – this will be considered in more detail at the next meeting.
2. Karen Lee also put forward that a community pharmacist has made contact with the group via Jacki Redfern, Nurse Practitioner at Muiredge Surgery, Buckhaven to ask if we would be interested in having him along to discuss “making the most of your community pharmacist”. The group felt that this would be interesting – Karen Lee to contact Jacki Redfern to discuss the pharmacists availability to attend our next meeting. [26/3/08]
3.Within the consultation document there was a note stating that “The Scottish Government now has an email alert system for consultations” – Colette Henderson has said that she will sign up for this and will let the forum know of any links or points that would be of interest to the forum.

Update on any other meetings
There were no reports of any other Non Medical Prescribers Meetings being held elsewhere in Fife or indeed further afield recently. However members present have agreed to feed back on any news updates they hear or any meetings that they attend.
Jill Logan asked the group about any members who sit on CHP Formulary / Drug groups. No one present [except for Jill Logan] sits on any such group, however all present felt that it was something that should be explored. Gill Dennes will ask Fiona Forrest what the situation is in the Dunfermline CHP – and feedback at the next meeting.
It was also felt by a number of people present at the meeting that it would be valuable to have speakers from the Scottish Government along to the forum meetings, not only to hear them present, but for them to see the group and take part in one of our meetings.
Our Logo
Ron Cunningham, Paramedic from the Scottish Ambulance Service has designed a letter head logo for the Fife Prescribers Forum. The group felt that our new logo was excellent – and thank Ron very much for all his hard work. This Logo will now be used when we respond as a group to any of the consultation documents, or when we are writing to ask speakers to come along to the meeting. Our thanks once again to Ron.

There is now a list for training of those who fit the criteria. Initially there will be a pilot group trained; more details will follow once confirmation has been received from NHS Fife.
Secondary Care Issues
Ann McKean, Paediatric Asthma Nurse, is currently exploring communication methods between primary and secondary care to ensure that all health care professionals involved with caring for any particular child have the most up to date information. Anne will be sending out letters containing details regarding medications and changes after a consultation. e.g. If a child attends a paediatric clinic and there are any discrepancies or any alteration to medication or care following the visit – primary care will be notified of the changes. If primary care alters this care they should contact Anne as she needs to know when changes are made so that her records can be updated.
Next Meeting Date
Next forum meeting date was arranged for 6th February 2008 – however will now be held on the 16th of January 2008 to enable group to respond to the latest consultation document.
Meetings 2008 / Educational & Forum
It was agreed that the forum meetings and educational meetings should both go ahead in 2008 – as they both address different needs for the Non Medical Prescriber.
The next meeting of the Forum will be on 16th January 2007
Educational Meetings for 2008 – it was felt that these were very important, and it is essential that these move forward and address the Non Medical Prescribers Needs.
Karen Lee to revisit the feedback forms – to look at topics, she has agreed to feedback at next meeting about content. Colette Henderson made the suggestion that we should look at poly pharmacy again – but have it condition related so that Non Medical Prescribers could attend session most relevant to them and their clinical practice / or e.g. elderly and look at multi diagnosis, multi prescribing, compliance etc for the age group rather than the condition. It was felt though that the needs of the different Health Care Professionals was very different i.e. the needs of Practice Nurses, Nurse Practitioners, Health Visitors, District Nurses, Pharmacists etc were all very diverse, and we need to look at alternative types of meetings, because one size will not fit all.
It was suggested by Heather Sheddon, and agreed by many that a Half Day course and then evening workshops may be well supported by Non Medical Prescribers.

Interviews for the New Nurse Prescribers Advisors Post have now happened, and we are waiting to find out who has been appointed, when they will start, where they will be based and exactly what their role is.
